Benefit Enrollment Center Readiness Assessment

Thank you for your interest in joining the network of Benefit Enrollment Centers (BECs). BECs help low-income seniors and persons with disabilities find and enroll in all the benefits programs for which they are eligible. The BECs focus on connecting people to these core benefits:

Medicare Part D Extra Help/Low-Income Subsidy (LIS)
Medicare Savings Program (MSPs)
Medicaid
Supplemental Nutrition Assistance Program (SNAP) 
Low-Income Home Energy Assistance Program (LIHEAP)

In the following survey, you will be responding to questions to help to determine your organization's readiness to implement the BEC program. Basic organizational information, such as staffing, volunteer management, average caseload, partnerships, and outreach efforts will be gathered for this assessment. Allow 10-15 minutes to complete this survey, you will receive a readiness report via email following completion. 

We encourage that you print a blank copy of the survey before beginning so that you are able to gather all of the supporting information necessary to complete the survey.
